| Fees and Appointments
My fee is $120.00 for a fifty-minute session.
Unless there is a prior arrangement, full
payment is required to be paid monthly. I accept cash or checks.
I will provide you with a financial statement at the beginning of each
month for the prior month's sessions. I do not bill insurance companies
directly but on the monthly statement I will provide the necessary diagnostic
and procedural codes for you to submit. You are responsible for payment.
Insurance reimbursement typically takes one to two months. Usually,
insurance companies send the check to me in which case I can sign it over
to you or apply the amount to the following month's balance.
How frequently we meet depends on a variety
of factors. Dialogue and ongoing review will allow us to consider
the level of frequency that will be most useful to you.
Once we decide to work together, it is
best to find appointment times that are workable for both of us, and will
be consistent from week to week. For the duration of our work together,
I expect payment for each appointment regardless of whether you have attended.
You are not charged for the sessions when I am unavailable because of a
preplanned absence or illness.
